Symptoms:

Usually symptoms of kidney failure appear late in the disease process. The common symptoms are:

  • change in urinary habits like increase in urine frequency & volume particularly at night
  • nausea, vomiting, headaches, dizziness (due to accumulation of toxic wastes in the body)
  • swelling in body parts due to accumulation of fluid
  • high BP
  • bone pains and anaemia due to deficiency of certain hormones produced and regulated by the kidneys

Dialysis treatment:

It is the major treatment option for kidney failure. It is the medical word for filtering waste products and removing fluids from the body which the kidneys are not able to remove.

There are two types of dialysis:

  1. Haemodialysis
  2. Peritoneal dialysis. Both HD and PD are equally effective depending on the patient’s condition and doctor’s evaluation.

TB is a global disease. Every year about 330,000 patients are die every year in the world India alone. TB can involve any part of the body, but Lung TB is the commonest as TB bacteria spreads by airborne methods (cough). Patients generally have symptoms like weight loss, loss of appetite, low to medium grade fever for more than three weeks.

Diagnosis of TB can be a very challenging task

  • Sputum examination for AFB (Acid Fast Bacilli) in typical patients. If these investigations are negative, then diagnosis can be very difficult as TB can be a differential diagnosis in virtually all infective diseases
  • In countries like India, where TB is rampant, many a times it is a diagnosis by exclusion
  • In spine, brain, bone and abdominal tuberculosis, CT scans of specific areas can be very helpful

Lasts for at least 6-9 months. In borne TB, it can take up to 18 months
With proper compliance, response to anti-TB drugs is very good and most patients recover completely
Not taking first line drugs properly results in drug resistance and emergence of MDR-TB (MTB resistant to Rifampicin and Isoniazide). In MDR-TB second line anti-TB drugs are used for nearly 18 to 24 months and unfortunately, not with good results

You may have TB:
If you have undiagnosed fever and cough for more than 3 weeks
If you have blood in your sputum

Also remember:
Always take complete treatment for 6-9 months
Stopping treatment midway, even if you feel well, can promote MDR-TB, a very difficult disease to treat
Never add one or two second line anti-TB drugs to a failing first line regime, as it will only promote MDR-TB, please consult an expert
DOTS treatment under RNTCP is available at several TB centre free-of-cost.
Today lack of finance is not a reason to treat TB effectively in DOTS era.

ULCER

Ulcers are sores on the lining of the digestive tract

Ulcers occur most commonly in the duodenum (the first part of the intestines)

Second most common part is stomach (gastric ulcer).

CAUSES

  • A type of bacteria called Helicobacter pylori cause many ulcers.
  • Acid and other juices made by the stomach can contribute to ulcers by burning the lining of the digestive tract. This happens when the body makes too much acid or if the lining of the digestive tracts are damaged in some way.
  • Physical or emotional stress can aggravate the ulcer in a person who already has it

Moderate amounts of hair fall out but it is rare to see large bald spots.

Symptom Of A Medical Illness

Hair loss can be one of the symptoms of a medical illness, such as thyroid disorder (such as hypothyroidism or hyperthyroidism), a sex-hormone imbalance or a serious nutritional problem, especially a deficiency of protein, iron, zinc or biotin. These deficiencies are most common in people on restrictive diets and women who have very heavy menstrual flow.

Hereditary Pattern Baldness

In men, hair loss may follow the typical male pattern (receding front hairline and/or thinning hair at the top of the head). We normally lose approximately 50 to 100 scalp hairs each day. If more than this is falling out it is a cause of concern. One may also notice that the hair is generally thinner and that one or more bald patches have appeared.

PREVENTION

Some forms of hair loss can be prevented by minimizing stress, eating a healthy diet and using sensible hairdressing techniques, and, if possible, switching to medications that do not cause hair loss. Hair loss from fungal infections can be prevented by keeping hair clean and by never sharing hats, combs or brushes with other people. Hair loss from hereditary-pattern baldness can sometimes be prevented by medication.

Dehydration can be defined as "the excessive loss of water from the body." Our bodies require a certain amount of fluid intake on a daily basis to function; the minimum is about equal to 8 glasses (one liter or one quart). Requirements vary with activity and age, but most active persons need two to three times this basic amount. Basic fluid intake serves to replace the fluids which are required to perform our normal bodily functions. If we take in less or lose more fluid than is needed, the end result is dehydration.

CAUSES

Excessive loss of fluid through the intestinal tract can happen when the intestine is "inflamed" or damaged, or when bacteria or viruses cause the lining of the intestine to produce more fluid than can be absorbed. A decrease in oral liquid intake may be due to nausea or loss of appetite.

SYMPTOMS AND SIGNS

A reliable clue to indicate dehydration is a rapid drop in weight in few days (or at times hours). A rapid drop of over 10% of weight is considered severe. Symptoms may be difficult to distinguish from those of the original illness, but in general, the following signs are suggestive of dehydration; increasing thirst, dry mouth, weakness or light head endness (particularly if worsening on standing), darkening of the urine, or a decrease in urination. Severe dehydration can lead to changes in the body's chemistry, kidney failure, and can even become life-threatening.

HEADACHE

Mostly everyone suffers from occasional headaches, and some can be quite uncomfortable. But most are only temporarily disabling.

Usually headaches are temporary and will go away on their own. However, don't be shy about contacting your doctor if the pain is bothersome. A physician should check out any headache that is severe, recurring or accompanied by a fever

Seriousnous of headache Not every headache requires medical intervention. Some headaches result from missed meals or muscle tension and can be taken care of at home. Other headaches are a sign of something serious and call for quick medical assistance. If you experience the following headache symptoms, you should seek emergency medical attention:-

Tension, migraine and cluster are types of headaches. Migraine and cluster headaches are types of vascular headaches. Physical exertion increases the headache pain in vascular headaches.

The blood vessels in the tissue surrounding the head dilate or swell, causing your head to throb with pain.

Cluster headaches are much less common than migraines, the most common type of vascular headache.

Cluster headaches usually strike several times in rapid succession - lasting weeks or months.

Cluster headaches are more common among men and can be extremely painful.

Diagnosis

Most headaches are not caused by serious conditions and usually can be treated with over-the-counter medicines. Migraines and other types of serious head pain may need prescription treatments and monitoring by a doctor.

Tension Headaches

Tension or muscle contraction headaches are the most common form of headache, and they are often associated with periods of increased stress

The pain associated with tension headaches is often steady and dull and can be felt in the forehead, temple area and back of the neck

People often describe tension headaches as feeling like a tight band is encircling their head

Although tension headaches can last for longer periods, they usually disappear after the stressful period is over

Tension headaches are usually not associated with other symptoms and there is no pre-headache syndrome as is often seen in migraine headaches. Tension headaches account for about 90 percent of all headaches

Sinus headaches Sinus headaches can be the result of a sinus infection or allergies. Often following a cold or flu, sinus headaches are caused by inflammation of the sinus passages, air cavities located in the bones behind and above your nose. As the sinuses become clogged or infected, the increase pressure causes your head to ache. The pain is usually severe and ongoing, beginning in the morning and becoming worse if you bend over.

Pain and pressure around the eyes, across the cheeks and the forehead

Achy feeling in the upper teeth

Fever and chills

Facial swelling

Both heat and ice are commonly used to relieve the facial pain of sinus headaches.

Migraine headaches vary from person to person, but they are usually characterized by severe pain on one or both sides of the head and often are accompanied by other symptoms. They may include nausea and vomiting, light sensitivity and distorted vision, dizziness, fever and chills.

Migraines can be triggered by any number of factors, which vary from person to person. Some people react to certain foods such as wine, chocolate, aged cheese, processed meats and caffeine are commonly mentioned. Use of caffeine and alcohol can also trigger headaches.


Note: If you have a lot of bad headaches, keep track of your symptoms, headache severity and how you handled the pain. Bring the record with you to the doctor.

The normal body temperature in human beings is 37oC or 98.6oF. When the body temperature is more than this normal level, the condition is called fever. Fever is not a disease. It is only a symptom of a disease. It is the reaction of the body to any type of infection. The increasing temperature indicates the level of severity of the disease causes.

Malaria, Typhoid, Tuberculosis, Rheumatic fever, Measles, Mumps
Respiratory infections like pneumonia and cold, cough, tonsillitis,
bronchitis etc

Simple tips to follow:

The person is to be kept in a well aerated room

After 20 years, almost all diabetics may develop diabetic retinopathy.
Duration of diabetes is important for development of diabetic retinopathy, not the severity
Incidence is more in females as compared to males
Severity of diabetic retinopathy is not related to levels of blood sugar
It is transmitted as a recessive trait without sex linkage

An eye specialist can diagnose diabetic retinopathy with the help of fundus examination of eye
For every diabetic whether he or she has no diabetic retinopathy.
Yearly follow-up of vision and fundus is must
Control your blood sugar.
There is increasing evidence to suggest that good initial control of diabetes delays the development of diabetic retinopathy
Control of hypertension is essential in all diabetic patients to prevent detrimental effects to retinal circulation
Anaemia should also be treated as its presence may worsen diabetic retinopathy.
So take diet rich in iron like green leafy vegetables and other iron supplements
Avoid fatty diet
Aspirin and dipyridamole decrease platelet stickiness and may have beneficial effects but use after consulting a doctor
Exercise at least thrice a week.
Do brisk walking
Avoid smoking and alcohol as they are harmful for a diabetic person

I. FORGET FIVE-A-DAY-THINK 15-A-DAY

There is more to know about how to breastfeed, as most new mother are clueless, it is best to learn about breastfeeding in advance.

Breast milk is high in nutrient quality, it has the right amount of fat, sugar, water and proteins that is required for baby’s growth and development.

Doctors are recommend exclusive breastfeeding for the first six months, with continued breastfeeding along with complimentary foods which are nutritionally adequate up to two years of age

 Brown bread, bajra-based dishes

Low-fat yoghurt

Idly and meals that have a high GL Try not to have ragi porridge..

 BRUNCH

Vegetable clear soup/spinach soup (without cream)

HOW PREVALENT IS GESTATIONAL DIABETES IN THIS DAY AND AGE?

Gestational diabetes is becoming increasingly prevalent among pregnant women owing to dramatic lifestyle changes which result in dietary changes.  It is a condition where a woman exhibits high blood sugar levels during pregnancy.  It is important to realize that pregnancy is a precious and critical phase in a woman’s life and the need to exercise and eat the right food throughout the nine months of pregnancy, is imperative at this stage. 

WHAT ARE THE RISK FACTORS ASSOCIATED WITH IT?

During pregnancy, when the mother’s sugar level is high, it interferes with the growth and maturity of the baby.  The last three months of pregnancy are crucial and if the sugar levels are high at this stage, it will lead to health complications in the new born baby.  The baby may develop prolonged jaundice, respiratory distress syndrome or Neonatal hypoglycemia (low blood sugar in the newborn).

After pregnancy, there is a 30% to 50% chance for the mother to develop diabetes later in life.

WHAT CAUSES GESTATIONAL DIABETES IN WOMEN? HOW CAN IT BE CONTROLLED?

There are no specific reasons behind the onset of gestational diabetes.  During pregnancy, several hormones are released inside the mother’s body.

The placenta, a temporary organ joining the mother and the fetus, releases sugar during pregnancy.  So the mother’s body which is already processing sugar from external sources, receives more sugar from here.  Therefore, it is important to control blood sugar levels during pregnancy.

This can be done only by eating quality food.  One of the myths associated with pregnancy is that eating a lot of food is good for the mother.  It is not quantity but quality that matters at this stage.  People do not know that fats and carbohydrates also release a lot of sugar.

WHAT ARE THE METHODS TO DIAGNOSE GESTATIONAL DIABETES?  ONCE DIAGNOSED, WHAT IS THE CURE?

It is important to go for pre-conception counseling in order to understand how to be healthy during pregnancy.  Since the placenta itself increases blood sugar levels.  It is called Glucose Screening Test.  Give mothers 50gms of glucose and conduct a blood test to examine how they reduce glucose levels.  If the readings are abnormal, conduct a Glucose Tolerance Test,  examine how efficiently 100 grams of glucose is broken down in the body.  If the mother is diagnosed with gestational diabetes, recommend healthy food and exercise.  Sometimes also recommend insulin injections.
